Alexandra Codrington, 16-year-old son of baronet Sir Christopher Codrington, disappeared from his home in Oxfordshire after breaking up with his girlfriend. An officer managed to reach him by mobile phone, and talked to him for about an hour, until he heard a gunshot.

Alexander's body was found by officers a short time after he was pronounced dead by paramedics at the scene. The Independent Police Complaints Commission has launched an inquiry into officers' involvement.

An examination will be taking place of the various officers involved, to see whether anything at all could have been done to prevent this tragic outcome.
